CITY OF OMRO COVID-19 VACCINATION CLINIC

The City of Omro has teamed up with the Omro Pharmacy through our "City Cares" program to provide a location to administer vaccinations to rural Winnebago County residents currently eligible for COVID-19 vaccinations. We are proud to say we opened the first mass vaccination clinic in the county and have given well over 1000 vaccinations in our first month and a half (as of 2/22/2021) in our small community. City staff set up the old Dollar General building and prepared the floors with help from the school. Pharmacy employees, city staff, retired doctors and nurses, volunteer first responders, and volunteer residents help to run and clean the facility each week. This is a huge undertaking, but worth the time it takes when we know that vaccinating our residents helps our businesses, hospitals, schools and citizens get back to the world we miss.

Please bring your insurance and drug cards with you to your appointment. Please bring your vaccination card to your 2nd appointment and keep it for your records after it has been filled out. If you are able, print and fill out the paperwork below and bring it with you for quicker service. We will provide the paperwork for those who cannot do so. Masks are required in the building. The pharmacy will call you to schedule your appointment when it is your turn. Please be patient as the waiting list is quite long and the supply of doses is limited at this time. Appointment times are subject to change or cancellation if delivery of doses is delayed. We encourage you to get on other lists around the county (see https://www.dhs.wisconsin.gov/covid-19/vaccine-map.htm) and with your doctor to ensure vaccination as soon as possible.
Please fill out BOTH pages of the paperwork for your first dose and only the 1st Consent & Administration page for your 2nd dose.
